The lever sign test of the thumb evaluates for carpometacarpal joint arthritis. The examiner stabilizes the thumb and grasps the first metacarpal just distal to the carpometacarpal (basal) joint, then applies a back-and-forth shucking motion in the radial and ulnar directions. A positive test is indicated by reproduction of the patient’s pain localized to the base of the thumb.
